I had the same problem once,the front pulley shaft has a flat washer that is bent up along flatside of of nut.you need to unbend it to tighten the nut well i did that,and it happened again.So i removed the washer altogether ,tightened the nut and never had the problem again.the nut is REVERSE threads!of course some will disagree with this,oh well.'G'
